Spontaneous carotid cavernous fistula complicating pregnancy.

چکیده
Carotid cavernous fistula is a well-documented but rare condition in pregnancy, about which there are a limited number of reports in the literature. We report such a case in a 41-year-old woman presenting with right-sided headache, proptosis, and diplopia at 37 weeks of gestation. She was subsequently diagnosed to have carotid cavernous fistula based on angiography. Embolisation was performed in the postpartum period. Carotid cavernous fistula has the potential of serious morbidity including visual loss and intracranial haemorrhage. It can be treated effectively by endovascular embolisation, which confers a good prognosis. Although headache is a common complaint during pregnancy, obstetrician should be aware of this condition if the clinical presentation is suspicious.

Central retinal artery occlusion associated with traumatic carotid cavernous fistula: case report.
Carotid cavernous fistulas are a rare entity occurring as result of head trauma and also spontaneously. The authors report a rare case of central retinal artery occlusion complicating traumatic carotid-cavernous fistula, resulting in severe visual acuity loss. Spontaneous closure of the fistula was observed. The possible mechanisms responsible for this complication are discussed.
متن کاملSpontaneous Intracerebral Hematoma from Transient Occult Carotid-cavernous Fistula : A Case Report
After the spontaneous relief of initial symptoms by traumatic carotid-cavernous fistula (CCF), paradoxical worsening of patient's condition can be followed. We present a case of a 60-yr-old man whose audible bruit from a traumatic CCF had completely disappeared. A few days later, however, the patient had spontaneous intracerebral hematoma with cortical venous drainage. Complete obliteration of ...
متن کاملIndirect carotid cavernous fistula presenting as pulsatile tinnitus.
This paper reports a case of spontaneous indirect carotid cavernous fistula that presented with pulsatile tinnitus, left-sided temporal headache and left-sided ptosis. The pulsatile tinnitus, its aetiology and investigation are discussed. The importance of pulsatile tinnitus is highlighted, with a discussion of carotid-cavernous fistulas. This case illustrates that clinically silent cavernous s...
متن کاملSpontaneous carotid-cavernous fistula with ophthalmoplegia and facial palsy.
Spontaneous carotid-cavernous fistulae are uncommon. They are seen mostly in association with hypertension, atherosclerosis or intracranial aneurysm (Bickerstaff, 1970). Facial palsy was noted by Walsh (1957) in a case of traumatic carotid-cavernous fistula which recovered in one year. Involvement of the third, fourth, fifth and sixth cranial nerves has been frequently noted (Walsh, 1957; Sattl...
متن کاملSpontaneous carotid cavernous fistula presenting only with cranial nerve palsies.
PURPOSE To discuss the differences in angiographic findings between cases of spontaneous carotid cavernous fistula with and without the classical triad of symptoms (pulsating exophthalmos, bruit, and conjunctival chemosis). METHODS With CT, MR, and angiography, we examined 12 cases of spontaneous carotid cavernous fistula, five of whom presented only with cranial nerve palsies. RESULTS In t...
